Referrals
Refer a patient to the clinic
Referrals are accepted by fax, through your EMR, or with the online form below. Our current wait time is 7–21 days for complete referrals, and referring physicians who belong to a Family Health Team (FHO or FHT) will not be negated for referring to the clinic.
For referring providers
Refer a patient in three steps
A referral from a family physician or nurse practitioner is required. Assessments and most procedures are covered by OHIP.
Complete the referral
Include the reason for referral, relevant history, current medications and any previous imaging.
Send it to the clinic
Fax to 416-256-0602 or send it directly through your EMR.
We contact your patient
We book the assessment and the same-day MRI together, so nothing is scheduled twice.

Every referral is recorded with the patient’s name, the date it was submitted and your questions, so the clinic team can follow up.
