The patient portal is the secure part of this website where patients and referring offices sign in to apply for treatment, send a question and follow a referral from the moment it arrives.
What you can do in the portal
Keep your details up to date
Save your contact details, date of birth, health card number and family doctor once, and they are ready the next time you apply or send a question.
Apply for treatment
Tell the clinic what you are looking for, where the pain is and how long it has been there. Your application goes straight to the clinic team.
Send a question
Ask the clinic team a question or send a self-referral from inside the portal. Replies arrive by email and are kept with your request.
Follow your referral
See where your referral stands, from received through to approved for treatment. The clinic aims to respond within 7–21 days for complete referrals.
For referring offices
Referring offices can sign in to send a referral and see every referral they have sent to the clinic, with its current stage.
Private by design
Your details are visible only to you and the clinic team, and the pages inside the portal are kept out of search results.
Questions about the patient portal
A referral from a family physician or nurse practitioner is still needed before an assessment can be booked, and the clinic aims to respond within 7–21 days for complete referrals. The portal is where you follow that referral, keep your details current and reach the clinic team without phoning.

- What is the patient portal?
- It is the secure area of this website where patients of the clinic sign in to keep their details up to date, apply for treatment, send a question to the clinic team and follow the progress of a referral.
- Who can sign in?
- Patients of the clinic and referring offices. Patients create an account with their email address, and referring offices can sign in to send a referral and see the referrals they have sent.
- Do I need an account before I am referred?
- No. A referral from a family physician or nurse practitioner is still required before an assessment can be booked, and referring offices can fax it to 416-256-0602 or send it through their EMR. An account simply lets you follow your referral and send questions once it is with the clinic.
- What can I do once I am signed in?
- You can save your contact details, health card number and family doctor, apply for treatment, send a question or self-referral to the clinic team, and read the clinic's replies in one place.
- Is my information private?
- Your details are visible only to you and the clinic team. Health information is never shown in search results, and the portal pages themselves are kept out of Google.
- What if I cannot sign in?
- Call the clinic at 416-256-0600 during opening hours, Monday to Friday, 9:00 am – 5:00 pm, or email info@uipc.ca and the clinic team will help.
